We are seeking a Virtual Registered Nurse to provide high-quality care to our patients remotely. In this role as a nurse coordinator for a preventive neurology clinic, you will be responsible for monitoring patients' health, and providing support to patients and their families to reduce their risk for brain diseases such as dementia, stroke, and other neurologic diseases via online platforms. You will also work closely with other members of the healthcare team to ensure that patients receive the best possible care and for our team to grow.

Responsibilities: 

• Recording  patient vital signs(when available online) 
• Documenting  a comprehensive medical history and providing treatment plan
• Documenting  lifestyle information (diet, exercise, stress, sleep, and mental activity,)
• Documenting medical history, family history, social history, and ADLs. 
• Conducting cognitive test, eg. MOCA (to be trained) 
•  
• Performing online physical / neurologic examinations
• Requesting diagnostic tests from different facilities
• Document medication adherence and treatments for patients as prescribed by physicians
• Monitor patients' health, lifestyle adherence and provide support to patients and their families via online platforms
• Collaborate with other members of the healthcare team to provide high-quality care
• Document patients' medical histories and treatment plans

Qualifications:

• Registered Nurse license in good standing
• Experience in a healthcare setting
• Strong communication and interpersonal skills
• Proficiency in using online platforms and technology
• Ability to work well in a remote team environment
• Commitment to providing high-quality care to patients
