The Caregiver assists residents with activities of daily living, administers medication as directed, assists in maintaining a safe and clean environment for residents, and assists with providing activity programming for residents.
Location: far-east Madison and Fort Atkinson 4-bed AFH, full-time and part-time, all shifts including weekends. Seeking a house manager.
1. Regularly communicate and develop relationships with residents, resident families, case Licensee / Administrator, referral sources, physicians, and staff.
2. Treat residents, families, visitors, and peers with respect and dignity.
3. Follow directions and works cooperatively with other staff members.
4. Maintain compliance with HIPAA.
5. Successfully complete training programs as directed and required by Chapter DHS 88, Wisconsin Administrative Code.
6. Is knowledgeable about and adheres to facility policies and procedures.
7. Assist residents with accomplishing Activities of Daily Living as directed in ISP.
8. Routinely review and be familiar with ISPs for residents under his / her care.
9. Perform medication administration as directed.
10. Assist with providing activity programming for residents.
11. Communicate with oncoming shift to ensure continuity of care is provided for residents.
12. Perform housekeeping and laundry tasks as directed.
13. Prepare and serves meals to residents.
14. Contribute to development of ISPs.
15. Participate in resident care conferences.
16. Monitor each resident on an ongoing basis for any potential change of condition.
