Organization Profile: 

Open Source Wellness (OSW), an Oakland-based 501(c)3 nonprofit founded in 2016, is dedicated to creating “Community as Medicine” (CAM) by partnering with communities and healthcare to deliver joyful, trauma-informed, and culturally relevant programs for health, wellbeing, and human connection. OSW partners with YMCAs, Federally Qualified Health Centers (FQHCs), and other clinical and community organizations. Utilizing culturally-relevant health coaches and peer leaders to support diverse and transdiagnostic populations, the CAM model sits at the intersection of physical health, mental health, and social health. Our work combats social isolation while uplifting the practices that underlie health and wellbeing: MOVE (physical activity), NOURISH (healthy meals), CONNECT (social support), and BE (stress reduction).  The organization is 70% revenue-based and 30% grant-funded.  For more information, please visit www.OpenSourceWellness.org
