
Job Summary:
We are seeking an enthusiastic and compassionate Physician Assistant to join our
Lifestyle Medicine Program within a Direct Primary Care (DPC) practice. The
ideal candidate is passionate about preventive care, patient empowerment, and
sustainable health solutions. They will play a vital role in delivering personalized,
evidence-based lifestyle interventions and fostering strong patient relationships,
all while working within a membership-based care model that emphasizes
accessibility, quality, and continuity of care.
Key Responsibilities:
Patient Care & Lifestyle Medicine:
• Provide comprehensive, patient-centered care with a focus on lifestyle
interventions, including nutrition, physical activity, stress management, sleep
optimization, and tobacco/alcohol cessation.
• Develop personalized health plans tailored to patients’ goals, medical conditions,
and lifestyles.
• Conduct wellness visits, chronic disease management, and health coaching
sessions in collaboration with the primary care team.
• Monitor patient progress, provide ongoing education, and adjust treatment plans
as needed.
• Design, implement, and monitor the Lifestyle Medicine Program, ensuring
alignment with the practice’s DPC model.
• Develop group visits, workshops, and educational resources focused on lifestyle
medicine topics.
• Collaborate with physicians, health coaches, and support staff to integrate
lifestyle medicine principles into the broader care model.
• Track program outcomes and gather patient feedback to continuously enhance
services.
Collaboration & Community Engagement:
• Build strong relationships with patients to foster trust and engagement in their
care plans.
• Partner with local organizations, fitness centers, and wellness initiatives to
create community-based health opportunities.
• Organize community events, webinars, or group classes promoting preventive
care and lifestyle changes.
Administrative & Documentation:
• Maintain accurate, timely documentation using the practice’s EHR system.
• Participate in team meetings, case discussions, and quality improvement
initiatives.
• Ensure compliance with DPC regulations and best practices.
Qualifications:
• Master’s degree from an accredited Physician Assistant program.
• Current PA licensure and certification (NCCPA) in New York State.• Minimum 2 years of clinical experience, with preference for experience in
primary care, preventive medicine, or lifestyle medicine.
• Certification in Lifestyle Medicine (or willingness to obtain within the first
year).
• Strong interpersonal skills with a patient-first mindset.
• Comfortable working in a DPC model, with an emphasis on long-term patient
relationships and proactive care.
• Excellent communication, leadership, and organizational abilities.
