We are seeking a dynamic and empathetic Health Coach to join our practice YashiCare. As a Health Coach, you will play a pivotal role in supporting patients with rheumatological conditions in adopting healthy lifestyle practices that complement their medical treatment plans. Your expertise in motivational coaching will empower patients to take charge of their well-being and make sustainable changes to improve their quality of life.
Conduct virtual health assessments to understand patients' medical history, lifestyle habits, and specific challenges related to their conditions.
Utilize active listening and motivational interviewing techniques to build rapport and establish a supportive coach-patient relationship.
Maintain accurate and up-to-date EMRs for all patients in compliance with regulatory standards.
Collaborate with patients to develop personalized lifestyle medicine plans that align with their rheumatology treatment goals and individual preferences.
Create actionable steps and realistic health goals that promote positive behavior change and enhance overall well-being.
Educate patients on the impact of nutrition, exercise, stress management, sleep, and other lifestyle factors on rheumatological health.
Provide evidence-based guidance and resources to empower patients in adopting healthier lifestyle choices.
Employ behavior change techniques to assist patients in overcoming obstacles, forming healthy habits, and sustaining long-term improvements.
Monitor patient progress and provide ongoing encouragement and support to foster motivation and adherence to lifestyle changes.
Communicate effectively with the healthcare team to exchange information and track patient outcomes.
Utilize virtual health technologies and digital platforms to conduct remote coaching sessions and provide patients with resources, tools, and educational materials.
Stay updated with the latest developments in rheumatology, lifestyle medicine, and coaching methodologies through self-directed learning and participation in relevant training programs.
Certification as a Health Coach from an accredited institution (e.g., National Board for Health and Wellness Coaching, International Consortium for Health & Wellness Coaching).
Strong coaching, motivational, and behavior-change skills.
Empathetic, compassionate, and supportive demeanor with exceptional interpersonal communication skills.
Ability to adapt to patients' individual needs and cultural backgrounds.
Commitment to promoting patient-centered care and advocating for patients' well-being.
Experience in health coaching, preferably with a focus on rheumatology and lifestyle medicine.
Familiarity with rheumatological conditions, their impact on patients, and common treatment modalities.
Proficiency in conducting virtual coaching sessions and using digital health platforms.
Health coaches will be able to learn how to guide autoimmune patients.
