Job Description

*This position is a hybrid role that requires in office on Tuesdays and Thursdays at 1200 Binz St Suite 1490 Houston TX 77004. Mon, Wed, and Fri are typically work from home.*

The Nurse Care Manager works closely with the population health team, primary care providers, and practice operations to ensure seamless transitions in patient care, address and achieve quality targets, and provide care coordination for Privia’s highest risk patients.

Primary Job Duties: 

  • Under direction of the Director of Population Health and Population Health leadership, work in collaboration with the Quality Manager and the Population Health Coordinators/Associates to achieve Quality targets
  • Manage a robust patient case load across various payers
  • Review patient non-compliance issues related to open gaps and conduct outreach to attempt to close them
  • Develop trust and build rapport with a select (high-risk/high utilizer) Privia patient population
  • Establish “call me first” relationship with your patients
  • Patient outreach - Transitions of care, Medication Reconciliation Post Discharge, Medication Adherence
  • Engage patients telephonically, in office, in facility, or where needed
  • Act as the liaison and advocate between the PCP/care center office staff and the patient, as well as with payer partner resources when appropriate/available
  • Assist patients in accessing care at the appropriate source
  • Have attention to detail for patients: medical, behavioral, social, and economic issues - work to mitigate these barriers
  • Schedule patients for diagnostic testing and follow up to ensure completion of testing
  • Develop relationships with the care coordination teams
  • Evaluate and utilize data from EMR to support patients needs
  • Maintain a courteous and helpful manner when working with patients, physicians, and associates
  • Participate in special projects and perform other duties as assigned

Qualifications

  • BS in Nursing (or equivalent) and at least 3 years of case management or care coordination experience.  
  • At least 5 years of nursing experience in a hospital, skilled nursing, long term care, or hospice setting preferred.
  • Medicare Advantage experience preferred.
  • Experience with triage preferred.
  • Active unrestricted Registered Nurse license in TX required. 
  • Must comply with HIPAA rules and regulations.
