Summary of Duties and Responsibilities:
The Plant‐Based Lifestyle Medicine Program (PBLM) is designed to help patients living with chronic cardiometabolic conditions adopt healthy lifestyle changes across the pillars of nutrition, physical activity, sleep, stress management, social connections, and avoidance of risky substances. Clinical criteria for referral to this program include atherosclerotic cardiovascular disease, hypertension, prediabetes, type 2 diabetes, and/or health concerns related to obesity. The Program Coordinator will oversee the day‐to‐day operations of the Plant‐Based Lifestyle
Medicine Program.
