Purpose of Position:
Under general supervision of the Medical Director of the Plant-Based Lifestyle Medicine (PBLM) Program, the Health Coach provides health coaching support (with emphasis on plant-based nutrition) to adult patients enrolled in the PBLM Program. This program is designed to help patients living with chronic cardiometabolic conditions adopt healthy lifestyle changes across the pillars of nutrition, physical activity, sleep, stress management, social connections, and avoidance of risky substances. Clinical criteria for referral to this program include atherosclerotic cardiovascular disease, hypertension, prediabetes, type 2 diabetes, and/or health concerns related to obesity. The health coach follows hospital and departmental policies and procedures and participates in all interdisciplinary activities as a member of the health care team.
Summary of Duties and Responsibilities:
1. A masters degree from an accredited college or university in public health education, education, public/community health administration, public administration or business administration, and one year of full-time satisfactory experience in developing public health education programs, including identifying target populations, conducting needs assessments, designing educational materials, planning educational presentations or workshops,
evaluating health education programs; presenting public health education programs; and/or counseling in areas such as communicable diseases, substance abuse, assault, sexual abuse, and/or family planning; or,
2. A baccalaureate degree from an accredited college and two years of full-time satisfactory experience as outlined in one (1) above.
