Purpose of Position:
Under general supervision of the Medical Director of the Plant-Based Lifestyle Medicine (PBLM) Program, the Health Coach provides health coaching support (with emphasis on plant-based nutrition) to adult patients enrolled in the PBLM Program. This program is designed to help patients living with chronic cardiometabolic conditions adopt healthy lifestyle changes across the pillars of nutrition, physical activity, sleep, stress management, social connections, and avoidance of risky substances. Clinical criteria for referral to this program include atherosclerotic cardiovascular disease, hypertension, prediabetes, type 2 diabetes, and/or health concerns related to obesity. The health coach follows hospital and departmental policies and procedures and participates in all interdisciplinary activities as a member of the health care team.

Summary of Duties and Responsibilities:

• Works as a member of an interdisciplinary team in providing supportive coaching for patients in the PBLM program on behavior change and the pillars of lifestyle medicine: healthful plant-based diet, physical activity, sleep, stress reduction, social connections, and avoidance of risky substances, with special attention to sleep, stress reduction, physical activity, and overall well-being
• Offers regular individual follow-up via in-person and/or virtual meetings that may include goal-setting, meal planning, and motivational interviewing
• Co-leads and assists in development of curricula for shared medical appointments focused on lifestyle change
• Documents patients’ progress including care plans, consultations, recommendations, and follow-up interventions in the electronic medical record.
• Coordinates exercise classes with exercise trainer/physiologist and participates in classes to provide staffing continuity for patients in the program
• Oversees flow of patients through the program, assigning patients to group class cohorts, helping track attendance, and working with program coordinator to ensure each patient has coordinated care.
• Maintains communication with local community leaders and groups on key projects, outreach, and courses related to plant-based nutrition and lifestyle medicine.
• Develops staff training programs on lifestyle medicine for to patients, employees, students, and clinical staff
• Assists dietitian and physician staff in the development and dissemination of culturally appropriate educational material to meet the needs of an ethnically diverse patient population.
• Performs related duties as required.

1. A masters degree from an accredited college or university in public health education, education, public/community health administration, public administration or business administration, and one year of full-time satisfactory experience in developing public health education programs, including identifying target populations, conducting needs assessments, designing educational materials, planning educational presentations or workshops,
evaluating health education programs; presenting public health education programs; and/or counseling in areas such as communicable diseases, substance abuse, assault, sexual abuse, and/or family planning; or,

2. A baccalaureate degree from an accredited college and two years of full-time satisfactory experience as outlined in one (1) above.

• Certification from health coach training program approved by National Board of Health and Wellness Coaches preferred.
• Certification in personal training, yoga instruction, and/or mindfulness/stress reduction preferred.
• Experience facilitating group classes preferred.
• Familiarity with plant-based cooking and meal planning preferred.
• At least 2 (Two) years of experience in health coaching preferred.
