Job Summary
The Registered Dietician (RD) Health Coach will work collaboratively with the full care management team as a key resource for 1:1 member education and health coaching. The RD Health Coach will be responsible for following through for receiving and sending referrals to the RN Care Managers, Social Workers, Wellness Coordinator, and Care Management Assistant to provide a coordinated, community-oriented, person-centered approach to HTA members in need of behavioral change to improve health status, reduce health risks and improve quality of life. Most work is centered around diabetes management, medical diet compliance, and weight loss assistance. Member interactions may be telephonic and in-person group coaching sessions with Medicare Advantage members. Successful candidates will be skilled in motivational interviewing, group facilitation, and engagement strategies. This position needs to have excellent knowledge of health, fitness, and nutrition. Other duties may be assigned as needed.
Major Work Activities
- Provide education to members facing challenges with weight loss, diabetes, and/or medical diet restrictions. Work hand in hand with RN Care Management to provide member-centric coordinated care services. Work with Social workers to identify community resources for members facing food insecurities. This education may be 1:1 or in group settings. Most work is telephonic; however, bi-weekly travel to provide group education is required. This person Is seen as a role model for a positive, healthy lifestyle.
- Assists clients in establishing effective, SMART health and wellness goals and works with clients to develop the most effective plan to achieve those goals.
- Utilizes health coaching techniques to achieve behavior change with clients/patients.
- Ensures timely, thorough, and compliant documentation at all times.
- Develops and provides group education programs at various community centers regarding health and wellness. Works hand in hand with the HTA chef to provide cooking demonstrations with nutritional information. Works collaboratively with HTA’s marketing to support Facebook and Instagram postings.

Requirements
Minimum Qualifications
Education
Bachelor's degree in diet/ nutrition or a closely related field
Licensure/Certification
- Register Dietician Licensure
- Successful completion of an accredited professional health coach training program.
- Annual Flu vaccine
- COVID-19 Vaccine
Experience
A minimum of four years of experience as a registered dietician.

- Excellent communication skills, both written and verbal.
- Good leadership qualities and enthusiasm
- Demonstrate courtesy, dignity, and respect to all individuals.
- Ability to work with a diverse group of individuals.
- Motivated to help others reach behavior change goals

- National Board Certified Health & Wellness Coach
- Specific Course Work in Health Coaching
Lifestyle Medicine Certification Preferred (or Required)
